Things to know for Your Eye Exam:

​ We need all patients to update their personal and medical information before their visit by accessing their Forms through their Patient Portal.
UPDATING PATIENT FORMS...

•  PATIENT PORTAL  to update Medical Information
  • First time users:
    • Open email we sent titled  "Portal Access", this has your username and now you will make your password to something you can remember.
    •  Click either link, Click reset password, Enter your username and click reset password again, now check your email (most likely the link will be in your spam folder), Follow the directions to reset your password.  You now have a username and password for you to access your portal at any time.​​
  • Established users (previous medical visits):
    • If your medical forms are not prefilled, please email or text us 321-724-2020 to have us prefill your information so all you need to do is update changes.
• FORMS TO REVIEW (signatures will be obtained at your visit)
• Authorization to schedule appointment and provide care to Minor Children

Things to bring:

• Insurance cards
• Photo id 
• Current glasses and sunglasses 
• Contact lens information, new patients only (the boxes or label on your current lenses will give the  information we need)
